Who Is Bear Paw KMA?


The Bear Paw KMA is a collaborative of agencies and individuals who care about kids that works with the community to coordinate services for youth with mental health concerns.  Staff includes a Parent Coordinator and Youth Coordinator who work directly with families on coordination services.  The Evaluation Technician coordinates all evaluation activities.

How Does KMA Work?


The KMA, with guidance from the family, assembles a team of individuals (family members, youth, natural supports, service providers and agencies) who come together and form an individualized plan of care for the youth and their family. This team develops, implements, and evaluates the plan over time. The plan usually consists of formal services, informal supports, community activities, and services aimed at maintaining the youth and family in the community. The family dictates the types and mix of services provided with those services being provided in the least restrictive environment appropriate for the youths needs.

Who Does KMA Serve?


The KMA serves youth, and their families, ages 0-18 who have a DSM-IV diagnosis or is suspected of having a DSM-IV diagnosis. Examples of DSM-IV diagnoses include, but are not limited to, Attention Deficit Hyperactivity Disorder (ADHD), Oppositional Defiant Disorder (ODD), Major Depressive Disorder, Bipolar Disorder, and Schizophrenia. The KMA can serve youth that are 19 years of age as long as the youth is currently enrolled in High School. The service area includes the entirety of Blaine and Hill Counties.

What Services Does KMA Provide?


These are a few of the services the Bear Paw KMA can provide:

  • Care Coordination/Case Management

  • Individualized, strength based plan of care

  • Home visiting as requested by the family

  • Advocacy for parents and youth

  • Assistance paying for non-reimbursable services (not paid by Medicaid/CHIP/Private Insurance)

  • Weekly parent and youth support groups

  • One point of contact for a child’s mental health needs

How can KMA Help My Family/Community/Agency?

  • Helps eliminate the burden of coordinating services for youth working with multiple agencies and may help alleviate some of the financial barriers.

  • One plan of care, developed by youth and family, receives better buy-in and reduces confusion, increasing the likelihood of success for the families and youth.

  • Services are based on the strengths of the youth and family, increasing the buy-in.

  • Information sharing among agencies is easier.

Referring Youth & Families into the Program:

Referrals can be made by youth serving organizations or by families in need of care coordination. 

For agencies, referral forms are available in PDF and Word form from the list below, or you can contact the KMA office.  Referral forms should be completed only with the consent of the family and an Initial Release of Information form must accompany the referral form.

Families interested in self referrals can contact the KMA office to set up a meeting.  All necessary paperwork will be completed with KMA staff and families working as a team.
